Login attempt max count

Hi, here's something that happened twice and is inconvenient.

I try to log in (with the correct username/password) but then I'm treated to a "you have exceeded the maximum login attempt number" message and a captcha from hell. Even though that was the first and only login attempt of the day! Nor was it preceeded by a failed one, however long ago in time.

At least, not by me. Should I be getting paranoid(er)? (EDIT: as in, did someone else try to log in to my username? is there a way to find this out?)
